The scope of work encompasses three broad areas of
responsibilities and services shared by all CHRs that are
the heart of the program. The services provided by CHRs is
variable. The CHRs have the ability to perform many
functions including:
First Aid, Verbal/Written Reporting, Transportation (of
patients, supplies, medications), Measurement of Vital
Signs, Advocacy, General Health, Assessment/Referral,
Communication, Teamwork.
The following are service codes used as guidelines of the
activities of the CHR:
1. Health Education.
2. Case Find/Screen.
3. Monitor Patient.
4. Case Management/Coordination.
5. Provide Emergency Patient Care.
6. Provide Patient Care.
7. Provide Homemaker Services.
8. Transport Patient.
9. Interpret/Translate.
10. Other Patient-Centered Services.
11. Obtain Training.
12. Provide Environmental Services.
13. Program Administration/Management.
14. Community Development
