Eyeglass Assistance

(Tribal 20% Funds)

Eyeglass assistance to tribal members will not exceed $75.00 for single lenses, $100.00 for bifocals and $125.00 for trifocals.  Priority will be in the following order: elderly, school children and other tribal members based on need. Those individuals who received assistance in 10/2008 to 9/2009 will not be eligible until 10/01/2009 and those who received assistance in Fiscal Year 10/2009 to 09/2010 will not be eligible until Fiscal year 10/2011. 

Operations and Maintenance

 

This program assists tribal members with services in plumbing (basic plumbing within the house), septic tank pumping and water testing (rural water only). Septic tank pumping and plumbing services depends on the type of services that are needed by the tribal member household.

 

Effective Oct.1, 2009

All clients requesting O&M services must

* Provide proof of residency (utility bill, driver’s license, etc.)

* Provide documentation of home ownership - must occupy the residence

* Provide proof of tribal enrollment (CDIB card)

These documents must accompany the application when applying for services. Applications will not be processed without above named items. 

Special Medication

(Tribal 20% Funds)

Tribal members are assisted with special types of medication prescribed by a physician. The amount of assistance is not to exceed a maximum of $100.00 per fiscal year.
