Alzheimer’s Disease Program Initiative

The Alzheimer’s Disease Program Initiative (ADPI) goal is to strengthen home and community-based systems by implementing sustainable dementia-capable person-centered care to Native Elders living with Alzheimer’s Disease or Alzheimer’s Disease Related Dementias and the individuals that care for them. Our three objectives will be to implement Music and Memory, deliver dementia education and strategic behavioral management through person-to-person consultations and to bring awareness and community education through presentations.

The ADPI Program serves any Native elder enrolled in a Federally recognized tribe, living within the ten SW counties of Oklahoma living in their own home who have cognitive decline. To be eligible you must:

• Be enrolled in a Federally recognized tribe
• Be 50 years or older
• Live at home
• Have Alzheimer’s Disease Related Dementias that affect daily living
• Participate in our Music and Memory Program
• Allow our Dementia Care Specialist to complete home visits
• Complete Pre and Post assessments
