Transitional Housing Application
The Tribal Victim Services-Set Aside Program’s goal is to establish a Transitional Housing Program and provide services to victims so they can obtain permanent safe housing. This program aids with relocation costs, utility deposits, transportation, and basic necessities.

In addition, families are provided referrals for individual or group counseling, legal counseling, and community resources. Victimization types include adult physical assault (includes aggravated and simple assault), adult sexual assault, adults sexually abused/assaulted as children, child physical abuse or neglect, domestic (IP) and/or family violence, elder abuse or neglect, sex trafficking, and/or stalking/harassment.

This application is used to determine whether you are eligible and whether this program can offer you the support and assistance you desire. The questions in this application are used solely as a way of establishing whether this program is a good fit for your needs and situation. You have the right to not answer any question you believe is not necessary to determine eligibility.

Please submit the application with the liability waiver and the release of information signed. Once we receive your application we will review it and contact you within 3 business days. If you are eligible, we will set up a time to meet in person or phone and discuss the steps of the process. This meeting will take place in a private location where you feel safe.
