PROGRAM INFORMATION AND ELIGIBILITY REQUIREMENTS:

• Response Household Assistance Program (RHAP)
  •  RHAP Application FY-2023
  • W-9 Form and Landlord Information Form are both needed when requesting Rental or Deposit Assistance and need to be filled out by the landlord or property manager.
  • Residence Verification Form is needed to verify that the client does reside at the stated address matching the utility bill or lease address that is being requested for services.

  •  Zero Income Verification Form is needed when applicant is declaring that there is no income being received by any household member at the time of application submission.

For applications that are received complete, please allow 10 working days for processing and eligibility determination.

TO REQUEST AN APPLICATION OR CONTACT THE ARPA DEPARTMENT STAFF

The program can fax, email, and send by U.S. mail any requests for applications. The option for any home visits will only be available to critical conditions (48 hour cut off notice) for the elderly and those with physical disabilities that reside within Caddo County limits.
